Laparoscopic cholecystectomy is a minimally invasive surgery to remove the gallbladder. The surgery is performed via small incisions.  Laparoscopic cholecystectomy is now the standard of care for diseased gallbladder. Using the camera and small instruments the procedure is completed under general anesthesia. It is one most common procedures that are performed laparoscopically. Approximately 20 million people in the United States have gallstones of these there are approximately 300,000 cholecystectomies performed annually. The most common indications for surgery are acute or chronic cholecystitis, symptomatic cholelithiasis, biliary dyskinesia, acalculous cholecystitis, gallstone pancreatitis, and gallbladder masses or polyps.

The gallbladder is located on the inferior aspect of the liver bed on the right side of the abdomen.  The surgery involves removing the gallbladder entirely with the stones. All patients undergoing surgery will need to be medically optimized preoperatively. Preoperative antibiotics are used 30 minutes of incision per protocol. The surgery is performed under general anesthesia.

After anesthesia and intubation, the laparoscopic cholecystectomy begins with insufflation of the abdomen using carbon dioxide. Next, four small incisions are made in the abdomen for trocar placement (supraumbilical x1, subxiphoid x1, and right subcostal x2). Utilizing a camera (laparoscope) and long instruments the gallbladder is retracted over the liver. This allows for exposure of the proposed region of the hepatocystic triangle. Careful dissection is carried out to achieve a critical view of safety. Once this view is adequately achieved, the isolated cystic duct and cystic artery are carefully clipped and transected. Electrocautery or harmonic scalpel is then used to separate the gallbladder from the liver bed completely. This completes the procedure and the specimen is retrieved via a 10mm incision. The patient is then taken to the postoperative recovery area after the surgery.

Complications after laparoscopic cholecystectomy are rare few people may experience diarrhea after cholecystectomy, which goes away after a few weeks or months in most people. The most serious complication is when structures near the gallbladder are damaged. One of these structures is the common bile duct, which is the main tube that drains bile from the liver and gallbladder to the small intestine. Damage to the common bile duct or other biliary ducts can cause leakage or obstruction of bile flow and has symptoms of abdominal pain, fever, and yellowing of the skin, known as jaundice. These symptoms can also be caused by a gallstone that is left behind in the bile duct. Bleeding from a blood vessel near the gallbladder can sometimes occur. Most bleeding stops on its own, but rarely, persistent bleeding requires intervention. After cholecystectomy, patients who experience worsening pain, fever, chills, nausea, vomiting, or jaundice should see a doctor.
