Surgery

In the TAPP approach, three ports are placed at the umbilicus and at the level of both flanks. After entering the abdomen, the peritoneum is incised, and this requires closure after mesh placement. The hernia surgeon opens a peritoneal flap that usually starts at the medial umbilical ligament and is incised laterally towards the anterior superior iliac spine. The hernia is identified and the sac is reduced. The hernia defect is covered with mesh after complete skeletonization of the spermatic cord and dissection of the peritoneum so that it can accommodate a large piece of mesh. The surgeon closes the peritoneal flap after mesh placement and this may be either done with sutures or tack fixation.

In a TEP technique, the ports are placed typically in a line from the pubic bone to the umbilicus. Preperitoneal dissection is performed and a peritoneal flap is created. The next steps of the surgery are similar to the identification of the sac and dissection of the peritoneum. The hernia is identified and the sac is reduced. The hernia defect is covered with mesh after complete skeletalization of the spermatic cord and dissection of the peritoneum so that it can accommodate a large piece of mesh. The surgeon does not need to close the peritoneal flap after mesh placement. 

Complications related to laparoscopic inguinal hernia treatment can be divided into operative complications and postoperative complications. Operative complications can be related to laparoscopic access (which possibly could be minimized with the TEP technique) and injuries to the surrounding structures of the inguinal region including vascular structures and the bladder. Patient education

Most patients go home on the day of or the day after surgery. Patients can return to eating their usual meals at home. Mild to moderate pain is common for a few days and can be managed with pain medications as prescribed. Dressings on the incisions can typically be removed the day after surgery, at which time patients can resume regular showers, but some incisions may require special care as instructed by the surgeon.
